All new student athletes will need to have this form signed prior to engaging any in programs, clinics, and facility usage.

Coaches and Parents automatically become volunteers for the Alma Jean Foundation. All minors need to be accompanied by a parent/guardian at all times.

Coaches are able to request for use of facilities if available. Please inquire by contacting us.
